What to Bring

We’ll email your intake forms ahead of your visit so you can complete them at your convenience. If you need help finishing anything in the office, we are happy to walk you through it. Just bring the following and we will take care of the rest.

  • A valid photo ID
  • Your insurance card or policy information, if applicable
  • A list of any current medications or relevant medical history
  • Completed intake forms

No Insurance?

We also offer a variety of convenient payment options and a discount Membership Plan for guests without dental insurance.

To ensure you can get the treatment you want, we also offer third-party financing through Cherry.

Cancellation Policy

We kindly ask that you give us at least 48 hour’s notice if you need to cancel or reschedule your appointment. Appointments cancelled or missed without notice may be subject to a fee. We understand that life happens, and we will always do our best to work with you.
